Admission requirements
Before an assessment.
- AHCA Form 1823, completed by the PCP
- Current medication list
- Psychiatric notes, if applicable
- Income statements
- Managed-care residents: the plan’s approval letter, plus the case manager’s name and phone number
Then an in-person assessment.
Clinical fit
Good fit, and not a fit.
Good fit: stable mental health diagnoses (including schizophrenia, schizoaffective disorder, bipolar disorder, depression, anxiety); mild memory loss; walkers, canes and wheelchairs with independent transfers; incontinence; CPAP.
Case by case: moderate dementia without wandering; oxygen; insulin (self-administered or via Home Health); catheter or colostomy; hospice for current residents.
Not a fit: secured memory care, elopement risk, two-person transfers or lifts, feeding tubes, IV medication, ongoing skilled nursing, current physical aggression, acute psychiatric crisis.
